Whole Person Care pilot project

Pathways logo iconPathways to Health + Home — the City of Sacramento’s Whole Person Care program — is the cornerstone of the City’s commitment to housing 2,000 homeless individuals by 2020. Individuals experiencing homelessness are often the sickest and most vulnerable people in our communities. The four-year pilot program is aimed at reducing unnecessary emergency medical services for the most vulnerable and high-risk Medi-Cal patients. A consortium of partnerships has been formed to coordinate care in real-time and evaluate health and housing outcomes.

The City of Sacramento is managing the pilot project The City and health care partners contributed $32 million, which was matched by federal funds for a total investment of $64 million over four years. Learn more about the pilot program.
