Registration Forms

Registration is based on a first come, first serve basis and some locations are currently experiencing small waiting lists. Please call the center of your choice for space availability.
To register your child in the 4th "R" Program, download PDF registration and medical consent forms (below) and:

• Drop off the registration and medical consent forms at the center at which, you wish to enroll your child.
• Mail them along with your registration fee to our main office at: 4th "R" Office, 5725 47th Ave., Sacramento, CA 95824.
• Complete the CEL application for free or reduced cost child care qualification
