Client Information FormThe Department of Parks and Recreation is now using new software to process class and activity registrations. Before you can get all the benefits of this new software, you must submit a Client Information Form (CIF) for each member of your household. We use this information to create an account in order that you may use the automated systems. Learn more about the new software, Automated Phone Registration, and Internet Registration. If you prefer, a PDF version of the Client Information Form is also available. You may type in your entries to complete the form. After printing the form, follow the instructions on the form to mail it or fax it to staff. After we receive the form, we will send your Client Barcodes and PIN via US mail. Red items are required. |
