Sacramento Fire Department personnel look forward to assisting you with your request. Please follow these steps to request fire and/or medical incident records:

TO REQUEST A FIRE INCIDENT REPORT

Fire Incident Reports are available through the City of Sacramento’s City Clerk’s Office. While a written request is not required, public record requests should be in writing so that City staff can more accurately identify the records sought and process the request more efficiently. Please be as specific as possible when making your request; include dates and times, if applicable, and any details regarding the information you are requesting. For your convenience, these requests may also be made by any of the following methods:

MEDICAL INCIDENT RECORDS Requests

In an effort to streamline the processing of medical incident record requests and enhance the Fire Department's ability to provide timely and exceptional customer service to those requesting records, the Department has now contracted with ChartSwap to fulfill these requests.

How to Request Medical Incident Records:

Requesting a record is fast, simple and secure. To request a record, simply visit and register on ChartSwap's site. There is a $30 processing fee due at the time of request.

What you will need to register:

  • Completed and patient signed form (online form through ChartSwap)
  • HIPAA Authorization to Release Medical Records
  • Valid I.D. (California Driver's License, California I.D., Military I.D. or U.S. Passport)
  • If the patient's signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.

Medical document pick-up is no longer available at the Fire Department.
