incident reports

Sacramento Fire Department personnel look forward to assisting you with your request. Please follow these steps to request fire and/or medical incident records:

Fire Incident Reports are available through the City of Sacramento’s City Clerk’s Office. While a written request is not required, public record requests should be in writing so that City staff can more accurately identify the records sought and process the request more efficiently. Please be as specific as possible when making your request; include dates and times, if applicable, and any details regarding the information you are requesting. For your convenience, these requests may also be made by any of the following methods:

Telephone: (916) 808-5002

Mail or in person:

City of Sacramento
Office of the City Clerk
New City Hall
915 I Street, 5th Floor
Sacramento, CA 95814

Please visit the City's Public Records webpage for more details. Should you have any questions or concerns, please contact the City Clerk’s Office at (916) 808-5002.

Download, print and complete the Medical Incident Report request form to the best of your ability. Be sure the request form is dated and signed by the patient. If the patient’s signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.

The cost for a Medical Incident Report is $5.00. Acceptable forms of payment are checks or money orders made payable to Sacramento Fire Department. Please do not send cash. Return or mail the request, with payment, to:

Sacramento Fire Department
ATTN: Custodian of Records
5770 Freeport Blvd, Suite 200
Sacramento, CA 95822

NOTE: If you intend to pick up medical documents, please notify the Custodian of Records at (916) 808-1347 well in advance to ensure they will be ready prior to your arrival.
